What Is Gummy Smile?
A gummy smile is a condition where, when smiling, the upper incisors and a portion of the gums are excessively exposed. This happens due to the hyperactivity of the levator labii superioris aleque nasi and levator labii superioris muscles, which are responsible for lifting the upper lip during smiling. As a result, the person’s smile reveals more of the gums than is typically desired.

What Are the Causes of the Condition?
The most common causes of a gummy smile are genetic factors, and they may include one or a combination of the following:

  • Small size of the upper teeth due to gum hypertrophy (overgrowth).

  • Short or overly mobile upper lip.

  • Overactivity of facial muscles in the area.

  • Excessive elongation and development of the maxillary gums.

  • Hypertrophy of the upper jaw, often linked to open-mouth breathing or thumb-sucking in childhood.

How Is It Corrected?

  1. Botulinum Toxin (Botox):
    Botox is one of the most effective treatments for a gummy smile caused by hyperactive facial muscles. By injecting Botox into the levator muscles, it relaxes them, which reduces their activity and stops the lip from rising too high, thereby decreasing the visibility of the gums when smiling. This not only corrects the gummy smile but also softens nasolabial wrinkles that may have appeared as a result of muscle hyperactivity.

  2. Myomodulation:
    Myomodulation is a modern approach used to treat facial asymmetry and gummy smiles. Rather than paralyzing muscles like Botox, myomodulation uses dermal fillers of varying densities, injected at specific depths and points in the face. The goal is to tone and strengthen the muscles, helping them function more naturally and in balance with other facial features. This approach can provide a more permanent solution to the problem without significantly altering facial expression.

Duration of Procedure:
The procedure usually takes 15–30 minutes, depending on the method used and the extent of correction required.

Downtime:
There is minimal downtime associated with both Botox and myomodulation treatments. After Botox, some mild swelling or bruising may occur at the injection site, which typically resolves within a few days. Myomodulation may involve some slight swelling or tenderness, but this usually resolves quickly as well.

When Does the Effect Occur and What Is Its Duration?
The effects of Botox are usually visible within 3–7 days after the procedure, with full results appearing after about 2 weeks. Botox typically lasts 3–6 months, after which the treatment can be repeated. Myomodulation results can be seen almost immediately after the procedure, with full effects taking 1–2 weeks to manifest, and results generally last from 12 to 18 months.

Pre- and Post-Care:

  • Before: It is recommended to avoid blood-thinning medications (such as aspirin and alcohol) a few days before the procedure to minimize bruising. Consult your provider about any pre-existing conditions or allergies to ensure the best results.

  • After: Avoid touching or massaging the treated area immediately after the procedure. Refrain from strenuous activity, excessive heat, and sun exposure for the first 24–48 hours. Follow any additional post-care instructions provided by your specialist.

When Not Recommended:

  • Not suitable for individuals with active skin infections, severe allergies to Botox or dermal fillers, or certain neuromuscular disorders.

  • Not recommended for pregnant or breastfeeding women.
